Ischaldirh only tends to appear when he's gone hunting bears, but Sherarax can actually be spawned with you in your normal form.

She does minimal damage and the only dangerous thing she has is her poison. What makes her annoying is that she has 4k hitpoints and 40 endurance. That means that killing her is almost impossible as she regains hitpoints faster than you can take them away. She is said to be the child of Scabies. Kaethos the village elder has a line describing her, but it is not used in current version. It goes like this:
