Hmm... I think the easiest way would be to make it a "black screen" event then. Maybe with a nice black and white picture of a bed with people obviously doing something under the covers in the background?

Hmm...

Maybe... A mischievous scientist (science being new to the world) who works on genetics comes to IVAN_World? He creates many mutant monsters (explained: carnivorous mutant bunnies, two headed moose etc.) in the process of his research, and is ready to branch his work to human subjects. But any old human will not do. He needs a child, one who has yet to become adjusted to the world. One whose cells are easy to tamper with. Seeing as you are deemed the "most valiant" or "most chaotic" or... you get the idea, he decides that your offspring will make a fine specimen, and while you are off visiting New Attnam (or an entirely new city altogether) he makes off with your firstborn. Then when you return home, you call for your child, but he/she never comes. During your search for him, you discover a note, detailing the reasons for his capture, "for the good of science". You have heard there was one who dabbled in unknown arts, and so you head off to find him, slay him (or capture?) and save your child.

It would be nice if the scientist had a monologue before the battle, trying to convince you that science is good and that some sacrifices must be made or other stuff along those lines. But of course, because you're a "barbarian" you shun science and attack him anyway. An option to spare his life, or end it would serve for the Lawful/Chaotic endings. Of course, Lawful should be made harder due to you having to, oh, I don't know, knock him out while his HP is critical?

Or maybe, for a more "dramatic" twist, there is something you must do earlier on the game in order to ensure the scientist's experiment does not succeed on your child. And if you don't complete that which you should've earlier... well then, that leaves you fighting your mutated child as the final boss. Of course, the mutant child goes out of control and crushes the scientist before the battle. This would serve as the x2 ending, as it is easier than the others.

Dungeon: A laboratory hidden inside a labyrinth of some sort?

Circumstance: Since Petrus was questionably "killed" for the better endings, he could reappear (sans nut) to take your firstborn as revenge and maybe to sacrifice to Mortifer, since Valpurus failed him.
Perp: Vengeful Petrus
Dungeon: A crater? Created by Petrus upon his return?

@chao: hence my wanting to make it do 10% of the total kill count in damage rather than the whole grand total. That would be completely outlandish in IVAN, due to the low hp scores. And it would be used similar to an explosion or broken striking wand, affecting all body parts. Did I mention these Tonberries are found towards the end of the game?

CLARIFICATION:
Grudge Damage = (Total Kills x 0.10)
Spread to all body parts.
Therefore if TKs = 100, damage = 10 spread to all body parts. And so on.
